Webform Requirements
To add a webform to any Choctaw Nation website, the following information is needed upon request. Please include within the related Workfront request as a Word document.
Required Information
- Form Name – provide the public name of the intended webform.
- Form Submissions Recipient(s) – provide email address(es) of any needed webform submissions recipient(s). If preferred, a spreadsheet can be provided to the RM/client after a webform has been closed instead via OneDrive.
- Form Fields – provide a list of webform “fields” by name. If a specific type is needed on a field, i.e. radio buttons, checkboxes, drop-downs, etc.; please specify.
- Required Form Fields – Indicate which fields will be “required” upon user submission to complete the form.
- Help Text or Descriptions – Provide any additional help text or descriptions a user may need to know when completing the webform.
Additional Options
- Schedule When a Form Should Open – A specific date and time can be set to open a webform. Provide the date and time if intending to use this option along with a description to users to indicate when the form will open.
- Schedule When a Form Should Close – A specific date and time can be set to close a webform. Provide the date and time if intending to use this option along with a description to users to indicate a reason why the form has closed.
- Limit Submissions – Limits can be set, such as for an event registration, that has a maximum capacity. Provide the capacity number if intending to use this option along with a description (for users) to indicate a reason the form has closed when capacity limit has been reached.
- Confirmation Message – Provide the text you would like the user to see on the confirmation page of this webform.
- Routing Rules – If specific form data is submitted, it can be routed to the proper form recipient. For example, a user has selected “Durant” instead of “Pocola.”
